What if I Need to Change my Card Information?

There are a limited number of changes to your medical cannabis card that can be made by our office. If changes are needed, call us first to discuss them. Many other changes need to be made by the Office of Medical Marijuana Use. Requests to replace a lost or stolen card will require the cardholder to submit a Change, Replacement or Surrender Request Form, along with a copy of his or her Florida driver’s license or identification card and a $15 replacement fee.
