- This form is for patients switching to KindHealth from another doctor.
- Your first visit must be made IN-PERSON at our MIAMI clinic. (By law, first visits to any new doctor must be made in person.)
- Telehealth is available for 7-month visits only AFTER the first in-person visit.
Although our usual hours are below, you should always call ahead.
Tuesday and Thursday, 11-2 PM; and Saturday, 2:30-6 PM.
- Complete each step in the health form below to update your history.
- Schedule your in-person visit date at our Miami clinic.
- Pay the initial 7-month renewal fee – $75 ($99 every 7 months afterward).
Protected Health Information (PHI)
The personal information we collect is necessary for you to apply to the State Registry. We upload the required identifying data to the Florida state OMMU registry to get your marijuana card. This info is considered PHI and although we store it on our local computer, we use it solely for the purposes of this business transaction. KindHealth does not share, sell, or give access to any third party, nor do we send it to or store it in the ‘cloud‘. I’ve included our privacy notice below.
